Commonwealth Home Support Program

The Commonwealth Home Support Program (CHSP) helps you stay independent and in your home and community for longer.

If you want to stay in your own home, but need some help with daily tasks, or require entry-level care, the Commonwealth Home Support Program may be able to help.

For most people, living independently in your own home as you get older is what you want. However, sometimes you may need support with some daily tasks that you are no longer managing as well on your own. For example, you might need someone to help you change a light globe, a tap washer, assistance with light gardening, minor home modifications or assistance with meals.

As you get older your own home may need to be modified for you to be able to stay home – these modifications are carried out by qualified tradespeople and are designed with the aim of meeting your changing needs.

To access Lithgow Information and Neighbourhood Centre’s CHSP Services, you must first call My Aged Care on 1800 200 422. My aged care helps you find the information you need about local services and your eligibility to receive services.

My Aged Care will also refer you to a local service provider or the service provider that YOU CHOOSE.

Meals on Wheels

Indulge in convenience and nutrition with Meals on Wheels—accessible to all! Enjoy the flexibility of subsidising through your CHSP funding, seamlessly billing to your existing Home Care Package, or opt for private payment to enjoy delicious meals delivered right to your doorstep. Our dedicated volunteers bring the choice of either hot or frozen meals to your door on the weekdays of your preference. Opt for the convenience of a supply of frozen meals, allowing flexibility without compromising on nutrition, or select hot meals delivered daily. Our low-cost, nutritionally balanced meals are freshly prepared in our kitchen at the Lithgow Information and Neighbourhood Centre's premises on Padley Street. For those who enjoy cooking but need a little assistance, we also offer fresh peeled/cut vegetable packs. Catering to health, cultural, or religious requirements, we specialise in crafting special meals, including options for diabetes management.

Maintenance

Home Maintenance & Modifications

From essential safety enhancements and accessibility modifications to gardening and lawn mowing services, we've got you covered. Our fees are negotiated based on the specific job, ensuring tailored solutions for your needs. You will need to secure the necessary approvals through My Aged Care to access subsidies, making quality services not just accessible but also affordable. Certain tasks may necessitate an Occupational Therapy (OT) report, and obtaining this might involve engaging alternative service providers.

The co-ordination and supervision of HMMS jobs are managed by LINC CHSP Services and the work is undertaken by experienced contractors. LINC CHSP Service provides a financial subsidy subject to a means test.

Flo’s Kitchen at The Community Café

Join us at Flo's Kitchen in our Community Cafe every Thursday from 12:30 pm to 2 pm. Our inviting cafe-style environment warmly welcomes CHSP eligible visitors and their guests for a light and tasty lunch, conversation and occasional activities or entertainment, all for a small fee. Our menu offers tea, coffee, or a hot chocolate paired with savory and sweet delights. Flo’s Kitchen at the Community Café is the perfect place to make new connections and friendships.

Community Lunches

Join the Community Lunch Bunch and embark on adventures with our social outings! Every Wednesday, immerse yourself in a blend of engaging activities and delicious lunches in our local and surrounding areas. Revel in the company of others and savour a delicious meal—all while we take care of the journey with provided transport to and from your home. Join the fun, create memories, and simply pay for your own lunch, along with a small fee for the convenient transportation. Come along and make Wednesdays your day of discovery and connection! Referrals for “Transport” and “Social Support Group” from My Aged Care are required to attend.

Social Support Individual

This social support program assists CHSP eligible individuals by providing volunteers or staff to help with tasks like shopping, bill payments, and companionship. Services are adaptable and based on individual needs, but availability may depend on volunteer schedules. To be eligible, clients need to have a My Aged Care approval for CHSP Social Support Individual. The focus is on promoting independence and well-being, particularly for older Australians.
