Commonwealth Home Support Programme (CHSP)
The Commonwealth Home Support Programme (CHSP) provides entry-level support services to help older people remain independent, safe, and connected within their home and community.
If you are finding some everyday tasks more difficult to manage, CHSP may be able to provide support to help you continue living at home for longer. Services are designed to support your independence, wellbeing, and goals, while helping you maintain your daily routines and quality of life.
Support may include:
- Meals and nutritional support
- Social engagement and community connection
- Home and garden maintenance
- Minor home modifications
- Practical assistance with everyday living
- Services are delivered in partnership with you and tailored to your individual needs and circumstances.
Client Contributions
Under the CHSP, clients are generally required to pay a reasonable co-contribution toward the cost of services they receive. Contribution amounts vary depending on the type of support provided and your individual circumstances.

Accessing CHSP Services
To access CHSP services, you will first need to register with My Aged Care. My Aged Care can provide information about available aged care services, assess your eligibility, and refer you to approved service providers, including LINC.

Food Services
Meals on Wheels
Enjoy delicious, chef-prepared meals made fresh in our local kitchen and delivered with care to your door.
At LINC, our Meals on Wheels service is about more than just a meal, it’s about comfort, nutrition, connection, and supporting people to live well at home. Our meals are freshly prepared in-house using quality ingredients, creating wholesome, home-style food that is both nutritious and satisfying.
Hot meals are delivered fresh on Mondays, Wednesdays, and Fridays by our friendly volunteers and team members, who provide a welcoming face and regular social connection. We also offer frozen meal options for added flexibility and convenience.

- Private fee-for-service arrangements
We cater to a range of dietary, cultural, religious, and health requirements, including texture-modified and diabetic-friendly options. For those who enjoy preparing their own meals but need a little support, fresh peeled and chopped vegetable packs are also available. Nutritious food, local care, and community connection, delivered to your door.
Maintenance
Home & Garden Maintenance
We provide practical support to help keep your home and outdoor spaces safe, tidy, and well maintained, supporting you to remain independent at home for longer.
Services may include:
- Lawn mowing and garden maintenance
- Minor household repairs
- Safety-related maintenance tasks
- Our team works with you to identify the support you need, with fees tailored to the specific services provided.
Services may be accessed through CHSP funding, your Home Care Package or Support at Home provider, or through private fee-for-service arrangements.
Home Modifications
Small changes to your home can make a big difference to safety, accessibility, and confidence at home.
We can assist with a range of home modifications designed to support mobility, reduce falls risks, and improve everyday living. Modifications may include the installation of handrails, ramps, grab rails, and other accessibility improvements.
Modifications may require assessment and approval through My Aged Care and, an Occupational Therapy (OT) assessment and report.
Our team can help guide you through the process and work alongside your provider to coordinate the supports you need.
The co-ordination and supervision of HMMS jobs are managed by LINC CHSP Services and the work is undertaken by experienced contractors. LINC CHSP Service provides a financial subsidy subject to a means test.
Social Support Groups
Stay connected, active, and engaged through LINC’s Social Support Groups and community activities.
Our programs are designed to support wellbeing, independence, and social connection by bringing people together in a welcoming and inclusive environment. Whether you enjoy getting out into the community, meeting new people, or participating in gentle activities, there is something for everyone.
Activities may include:
- Social outings and day trips
- Group exercise and balance classes
- Community lunches and morning teas
- Special events, information sessions, guest speakers and celebrations
- Group activities and social connection opportunities
- Transport to and from activities may also be available for eligible participants.
Our Social Support Groups provide a wonderful opportunity to build friendships, stay active, and enjoy meaningful experiences within the community.
To access subsidised services through My Aged Care, referrals for “Social Engagement” may be required. Our team can assist you in understanding the referral process and available supports.
Social Support Individual
