Disability Support Coordination Officer

LINC is seeking a friendly and compassionate individual who has a passion for working with young people and adults with a Disability to provide Support Coordination by linking with a wide range of service providers to provide assistance to reach their goals.

The successful person will have a Degree in relevant Community Services area.  This role will deliver a service to residents within the Lithgow LGA area.

Essential attributes to be considered for this role are:

6.1      Degree in Social Work, Psychology or Human Services and/or equivalent level of knowledge and experience in a relevant sector.

6.2      Previous experience and knowledge to work competently alongside people, their family and/or carers in their local community to build their capacity to participate in the community and manage some or all aspects of their plan.

6.3      Demonstrated understanding of the required supports, challenges and need for person centred active support of people with disabilities.

6.4      Ability to communicate complex processes to people with intellectual disabilities

6.5     Widespread Case Management experience and ability to liaise effectively with a wide range of participants and service providers.

6.6      Excellent written and verbal communication skills including the preparation of reports and submissions.

6.7     Demonstrated knowledge of the Disability Services Act and the Disability Standards and its application within Disability Services.

6.8    Demonstrated knowledge of current legislation, regulations and other agreements relating to WHS and NDIS.

6.9      Demonstrated leadership skills including ability to provide staff with clear sense of direction and inspiring others to perform to the best of their abilities.

6.10    Demonstrated experience in managing and developing external stakeholders to achieve the desired outcomes.

6.11   Previous experience in managing priorities and working remotely, to keeping strategic deadlines and follow-ups in timely manner.

6.12    Knowledge and demonstrated ability in the NDIS planning and funding allocations

6.13    Current National Police check and Working with Children Check

6.14    Current Driver’s Licence.

This is this is a newly emerging business area for LINC.
