The Commonwealth Home Support Programme (CHSP) helps you stay independent and in your home and community for longer.
If you want to stay in your own home, but need some help with daily tasks, or require entry level care, the Commonwealth Home Support Programme may be able to help.
For most people , living independently in your own home as you get older is what you want. However sometimes you may need support with some daily task that you are no longer managing as well on your own . For example you might need someone to help you change a light globe , a tap washer , assistance with light gardening , minor home modifications or assistance with meals.
As you get older your own home may need to be modified for you to be able to stay home - these modifications are carried out by qualified tradespeople . are designed with the aim of meeting your changing needs.
To access Lithgow Information and Neighbourhood Centre's CHSP Services , you must first call My Aged Care on 1800 200 422 . My aged care help you find the information you need about local services and your eligibility to receive services.
My Aged care will also refer you to local service provider or the service provider that YOU CHOOSE.

Doreen's Place - Lithgow Aged Overnight Respite Service

The Australian Government Department of Social Services provides funding to complement the day program at Doreen’s Place, specifically targeting the carer of a person with an My Aged Care assessment of Dementia or Alzheimer’s.
Doreen’s Place overnight respite program enables 1-2 persons to stay overnight between the two day visits (Thursday night).
Doreen's Place - Lithgow Aged Day Program Service

Doreen’s Place is a day centre for frail aged people with priority given to people with dementia including Alzheimer’s. The service operates on a Thursday and Friday in a family home-like environment which provides appropriate socialisation for a small number of people and respite for their carers. Transport to and from the service can be provided if required within the city limits.
Community Lunch and Flo's Kitchen

We arrange supported social outings for CHSP eligible people who are socially isolated. These outings combine social activity with a tasty lunch at various venues around the surrounding district.
They are held every Wednesday and are a good opportunity to enjoy the company of others whilst enjoying a good meal. Careful consideration is given to the cost of these outings and the safety of the venue. Transport is provided to and from your home which may incur a small fee.
Flo’s Kitchen 6354 5914
Flo’s Kitchen meets every Tuesday for morning tea. The café style environment welcomes our CHSP eligible visitors to enjoy a comfortable and relaxing cuppa with friends.
For a small donation you will enjoy home cooked savoury and sweet treats and a cup of tea, cappuccino or hot chocolate.
If you require transport to and from LINC we can pick you up and drop you back home.
Lithgow Aged Social Support

LINC CHSP Service provides volunteers or a staff member to assist CHSP eligible people with activities that you may not otherwise be able to do without assistance. This may include shopping, paying bills, companionship, reading or writing letters etc. This service can be adapted to meet the needs of the individual and depends on the availability of a volunteer.

Commonwealth Home Support Program ( CHSP ) Home delivered Meals are flexible and designed to meet your changing needs. Our volunteers deliver hot or frozen meals to your home on any weekday that you choose. Some people find it more convenient to order a supply of frozen meals instead of being home every day to receive hot meal .
Meals are low cost , nutritionally balanced and cooked fresh in our kitchen on the Lithgow Information and Neighbourhood Centre's premises, Padley street, Lithgow . If you like to cook but cannot cut up vegetables we can deliver fresh peeled / cut vegetable packs .
We can provide special diets for health reasons , including diabetes , or for cultural or religious reasons .
Home Modifications and Maintenance Service

Please note that all Home Modification work must be referred by or to an OT.
Occupational Therapists are employed by your Local Area Health Service.
Minor modifications may include:
- supply and installation of handrails or grab rails
- supply and installation of handheld showers
- access modifications such as ramps, step wedges, easy steps and paths to suit your individual needs

Home Maintenance 6354 5908
Lithgow Information and Neighbourhood Centre's Home Modifications and Maintenance Service provide some maintenance work relating specifically to safety and access for example repairing a set of stairs that may not be suitable for safe access.
As part of Home Maintenance Service we offer a Gardening Service. This service will incur a fee which will be negotiated depending on the size of the job.
The co-ordination and supervision of HMMS jobs are managed by LINC CHSP Services and the work is undertaken by our own experienced builder and/or qualified trades people. LINC CHSP Service provides a financial subsidy subject to a means test.
