LINC Disability Services can offer NDIS Plan Support Coordination to assist in strengthening a participant’s life choices based on the principle of putting people with a disability, their families and carers at the centre of the decision making process.
To enhance consumer outcomes and maximise the social and economic involvement of participants, certain experience and expertise is required to provide NDIS funding support; LINC’s NDIS Plan Coordinator can attend your planning meeting with you to ensure maximum plan benefits are received for your needs, whether you are time limited, low level or specialist coordination of supports.
An NDIS plan could include, but is not limited to;
* Financial management of your funding support
* Connecting you with informal, mainstream and funded supports to help strengthen your capacity and resilience and,
* Address any barriers you may be experiencing including providing experience and sourcing and linking with the right people to meet your needs and goals.
